The Leadership in Fitness Excellence (L.I.F.E.) award, co-sponsored by the Indiana Fire Chiefs Association and Public Safety Medical, recognizes the exemplary efforts of Public Safety Departments to improve the health and well-being of their personnel through wellness/fitness programs.

 

Three finalists will be selected from the nomination forms by a team of representatives from Public Safety Medical, Indiana Fire Chiefs Association, and the Indiana Association of Cities and Towns.  

The purpose of the L.I.F.E. Award is:

• To foster awareness of quality health, safety, and wellness programs
• To identify model programs and outstanding practices with measurable results
• To educate other first responder departments on the value of wellness and fitness programs

Winners receive:

• Recognition at the annual Indiana Emergency Response Conference 
• Cash and valuable prizes
• Crystal trophy for winner and plaque for runners up
• Recognition for best practices and model programs

Instructions:  Fill out the nomination form in its entirety.  The form should be typed in a double-spaced format.  The fire chief or chief executive officer must sign the form.  All programs being highlighted on the nomination form must have been in place for at least one year prior to nomination.  All forms must be submitted by Wednesday, August 3, 2011.
